Calculation Formula Sheet
• Desired dose ÷ dose on hand × quantity on hand = amount to administer.
• Quantity = amount per dose × doses per day × days.
• Days supply = quantity dispensed ÷ amount used per day.
• mL/hr = total mL ÷ hours.
• mg/kg dose = ordered mg/kg × weight in kg.
• 1 g = 1000 mg; 1 mg = 1000 mcg; 1 L = 1000 mL; 1 tsp ≈ 5 mL; 1 tbsp ≈ 15 mL.
• Use a leading zero for doses less than 1; avoid trailing zeros.
