Core Concept
The International Board Certified Lactation Consultant (IBCLC) credential, awarded by the International Board of
Lactation Consultant Examiners (IBLCE, established 1985), certifies that a clinician can provide skilled, evidence-based
care across the full spectrum of lactation — from preconception and pregnancy through weaning, and in normal, high-
risk, and complicated situations. It is the only globally recognized, NCCA-accredited lactation credential, held by more
than 39,000 practitioners in 137 countries. Candidates study this material because the IBCLC is positioned as an allied-
health clinician on the maternal–infant care team: the exam tests not isolated facts but clinical judgment — taking a history,
examining the dyad, building and documenting a care plan, and protecting breastfeeding within an ethical and legal
framework.
THE EXAM BLUEPRINT IN ONE BREATH
175 multiple-choice items in two parts (Part II is image-rich), each item mapped to one of 7 disciplines and one of 11
chronological periods, all oriented toward the tasks of building a care plan. Master the dyad across time, and you
master the test.
Key Concepts
1. Physiology & endocrinology of lactation
Definition: The hormonal and anatomical machinery of milk production — secretory differentiation in pregnancy
(Lactogenesis I), secretory activation after birth (Lactogenesis II, "milk coming in"), and the supply-and-demand
maintenance phase (galactopoiesis).
Why it matters: Almost every clinical problem — low supply, oversupply, delayed milk, engorgement — is a deviation
from normal physiology. You cannot fix what you cannot explain.
Key principle: Prolactin drives milk synthesis (volume); oxytocin drives milk ejection (let-down). After Lactogenesis II,
control shifts from endocrine (hormone-driven) to autocrine (local, demand-driven via the Feedback Inhibitor of
Lactation). Removed milk signals more milk.
2. Assessment of the breastfeeding dyad
Definition: Systematic evaluation of mother and infant together — maternal breast and nipple exam, infant oral anatomy
and tone, positioning, latch, and milk transfer (audible swallowing, pre/post-feed weights, output).
Why it matters: The IBCLC's signature skill is reading the dyad in real time and acting on it. The exam's image items
reward you for seeing what is actually happening at the breast.
Key principle: Assess the dyad, never one member in isolation. A "fussy baby" and a "low-supply mother" are often two
descriptions of the same transfer problem.
