Service Line for Home Health Services
Proposed Nursing Service Idea
The home care industry is undergoing a significant transformation.
Understanding the importance of home health care in the United States starts with
acknowledging the larger changes in demographics and health care preferences
throughout the nation. The spike was mainly driven by the aging baby boomer
demographic. Now, many older individuals require various types of home-based
assistance (Malana VanTyler, 2025). The home health services will provide resources
for the management of chronic diseases, meet the healthcare needs of patients in
rehabilitation, deliver healthcare services to elderly individuals, offer palliative care, and
assist post-hospital stable patients. This service line will decrease the necessity for
superfluous hospital stays and provide the necessary supervision to patients who meet
the criteria.
To successfully implement this service line using a diverse and interprofessional
team, the transformational leadership approach will be applied. Transformational
leadership promotes collaboration, inspires staff toward a shared vision, and
encourages innovation—all essential in creating a high-functioning, patient-centered
home health team. Home healthcare services represent an interdisciplinary approach to
patient treatment, engaging professionals with specialized knowledge. For instance,
physical therapists focus on enhancing muscle and bone strength, as well as improving
movement. Occupational therapists help patients maintain their independence in daily
activities. Speech therapists help with safe eating and swallowing, and other healthcare
services professionals participate in the conversation surrounding patient care. The
effective leader must possess cultural competence, demonstrate qualities of servant
leadership, and exhibit transformational leadership skills. The ideal candidate must
possess qualities such as empathy, active listening, open communication, shared goals,
and mutual respect, as well as the ability to motivate and uplift both employees and

services includes registered nurses, therapists specializing in physical, occupational,
speech, and behavioral therapy, as well as home health aides, along with dietitians and
primary care providers. Additionally, change management skills will be used to guide
staff through the transition as new workflows, technologies, and patient care processes
are introduced. By clearly communicating the purpose and benefits of the new service,
addressing resistance, and providing training and support, the leader will help staff
adapt successfully to new roles and expectations.
This service line will utilize mobile nurses to visit patients within the first 24 to 48
hours following their recent discharge from the hospital or referral from primary doctors.
The purpose of these visits is to ensure that patients have received their medications
and are adequately educated about them, that their appointments are scheduled, and
that they have the necessary transportation to attend these appointments. Additionally,
the nurses will conduct a health assessment to confirm that patients are progressing
appropriately according to their plan of care.
Importance
Chronic diseases affect a large percentage of the population in Orange County
and surrounding areas here in California. There is about 36% of adults in Orange
County report at least one chronic condition, compared to higher rates in some other
regions (Californians with the Top Chronic Conditions: 11 Million and Counting, 2015).
According to a 2022 community health needs assessment: among Medicare
beneficiaries in OC (age 65+), about 56.4% had hypertension, 53.4% had
hyperlipidemia, 34.1% had arthritis, and 26.5% had diabetes (UCI Health, 2022). Home
healthcare services will play a crucial role in an integrated care system, providing
advantages not only to the hospital but also to the Orange community. Residents in
Orange County who stand to benefit from this initiative encompass seniors aged 65 and
above, patients undergoing rehabilitation, stable post-surgical patients requiring
monitoring and guidance to facilitate their recovery, as well as individuals with chronic
illnesses, including heart disease, chronic kidney disease, liver disease, and diabetes.
