1. Wandering Pacemaker
Irregular Rhythm
P wave shape varies
Atrial Rate less than 100
Irregular Ventricular Rhythm
Pacemaker activity wandering from the SA Node to
nearby Atrial Automaticity Foci
2. Multifocal Atrial Tachycardia
Irregular Rhythm
P wave shape varies
Atrial rate exceeds 100
Irregular ventricular Rhythm
3 or more atrial foci are involved.
-COPD and digitalis toxicity
3. Atrial Fibrillation
Irregular Rhythm
Continuous chaotic atrial spikes
Irregular Ventricular Rhythm
No P waves, just erratic spikes
4. Wandering Pacemaker
Irregular rhythm with P waves visible. Presence of P
waves eliminates A-fib.
-P waves are not identical, and rate does not gradual-

ly increase or decrease, so it is not sinus arrhythmia.
-Rate is less than 100- rule out MAT
-P waves are different shapes c/w WP.
5. Atrial Escape Rhythm
Atrial focus escapes overdrive and becomes domi-
nant pacemaker
P' waves are not identical to P waves since the rhythm
originates in the atrial focus rather than SA node.
-Slower rate observed (60-80)
6. Junctional Escape Rhythm
Occurs s/p Sinus Arrest and if atrial foci also fail.
Rate is 40-60
-Usually conducts mainly to the ventricles, producing
a series of lone QRS complexes.
**Because each Junctional Automaticity focus is lo-
cated within the AV Node, each pacing stimulus there
will conduct to the ventricles as expected, but the
paced stimuli may also depolarize the atria from be-
low producing inverted P' waves with an upright QRS.
This can appear before, after, or buried in the QRS.
7. Ventricular Escape Rhythm
Occurs when a ventricular automaticity focus is not
stimulated by paced depolarizations above.
-Inherent rate is 20-40
