◉ Patient Instruction During NGT Insertion.
Answer: Instruct patient to touch their chin to their chest, encourage
patient to sip water through a straw or swallow.
◉ NGT Tube Securing.
Answer: Secure loosely to the nose until the placement is checked,
after confirmation of placement secure tube with tape, skin barrier
to nose and secure to patient's gown.
◉ NGT Removal Procedure.
Answer: Verify order for removal by HCP, perform HH and put on
PPE, identify the patient, explain procedure and rationale for
discontinuation of tube.
,◉ NG Tube Removal Procedure.
Answer: Raise bed to comfortable working height and assist pt to
30-45-degree semi fowlers position. Place a towel or pad on
patient's chest. Give tissues and have an emesis basin in reach. Put
on gloves, discontinue suction, and separate tube from suction.
Detach tube from gown and carefully remove tape on face. Check
placement and flush with 10ml of water or NS or 30-50ml of air.
Clamp tube with fingers, instruct the pt to take a deep breath and
hold it. Quickly/carefully remove the tube while the patient holds
their breath. Coil the tube on the pad/towel as you remove it.
Dispose of tube, per policy, remove gloves, do HH and offer oral care
to patient. Remove equipment, measure the amount of NG drainage
in the suction container.
◉ Nursing Actions to Maintain Tube Patency.
Answer: Check placement, keep HOB elevated to 30-45 degrees,
Irrigate with NS, check all connections, make sure suction is
working.
◉ Confirmation of NG Tube Placement.
Answer: X-ray is best confirmation, obtaining PH, visual assessment
of aspirate (color), auscultation of air blous-unreliable.
◉ Enteral Therapy Implementation Procedure.
Answer: *always check placement* PH <4 gastric >6 intestinal >7
lung.
, ◉ Continuous Enteral Feeding.
Answer: Per MD order, infuse via pump, ml/hr. Isotonic formula over
8-12 hours as tolerated, usually runs 10-40ml/hr. Fill bag with
appropriate amount/type of formula, hang bottle (change up to 36
hours), prime tubing and set rate. Flush tube with 30-60ml H2O,
connect tube and hit start. Assess every 4 hours. Check gastric
residual, flush with 30cc H2O.
◉ Intermittent Enteral Feeding.
Answer: Per MD, Toomey syringe bolus, at intervals. Isotonic
formula over 20-30 mins 2.5-5ml/kg 5-8x per day. Insert syringe
without plunger into end of feeding tube. Flush with 30-60cc, pinch
of kink tubing before syringe empties, fill with formula and infuse
slowly. Keep adding formula before any air gets into syringe. Use
plunger if infusion by gravity is not working. Follow with 30-60ml at
end of feeding, clamp tube and secure.
◉ HOB Positioning for Decreased LOC Patients.
Answer: Position the HOB between 30-45 degrees with decreased
LOC patients, keep HOB elevated 1 hr after BOLUS feeding. Assess
the insertion site of tube, and flush tube before, after and between
feedings and med administrations.
◉ Medication Administration Process.
