TASK A
A1. Assessment overview Type: Case Study
Purpose: A summative performance assessment will be done to evaluate if the
student can demonstrate what they have learned during the length of the course. The
reason for these types of assignments is for the student to obtain information relating
to the course topic. This assessment type will allow the instructor to evaluate how
well the student is doing and how much information the student has retained. The
student will be given the case study questions and use their critical thinking skills to
answer them appropriately.
Expected Outcome: The summative performance assessment is a great tool to
evaluate the understanding of the content by the student. Students are required to
make an 80% or better to show competency of the material.
A2. Module title Introduction to Community & Public Health Nursing
Course objectives After completing this course, the student will be able to:
1. Examine the social determinants of health based on a specific population and
how it impacts their community
2. Determine effective local, regional, and national strategies for improving
policies that affect social determinants of health.
3. Implement a plan to improve patient care based on their social determinants.
Student learning outcomes At completion of this module Student will be able to:
1. Identify social determinants of health as well as create an improvement plan
for the community.

A2a. Assessment Activity Alignment
, Course Objectives 1-3.
The Summative assessment makes students use their critical thinking skills and apply
information that they learned in the course. The student must be able to demonstrate
comprehension of material to be able to pass the assessment. In this assessment
students will examine the social determinants of health and how that can affect their
community. Students will explain what strategies should be used to decrease the
spread of disease and be able to create a plan to improve patient care based on the
social determinants that were discussed. This task will require that the student
understands the role of the BSN nurse within the public health care setting and
determine actions within the nurse’s role.
Student Learning Outcomes 1-2.
The summative assessment aligns with each student learning outcome because this
assignment requires student to use their critical thinking skills when answering
questions to a case study. With this case study assignment, students will examine the
spread of disease and how it negatively effects communities. Students will also look
at how social determinants of health relates to the patient. They will apply what they
have learned and work towards discovering what the BSN nurse’s role is in public
health.

Which is indicating that the student can recall or remember the information that has
been taught.
A4. Item Types Case Study with 10 Open-Ended Questions
The item type that I chose for the summative assessment is a case study that has ten
open ended questions to go along with it. Students will incorporate the impact of
social determinants of health, compare the differences of
communicable/noncommunicable diseases and discuss the nurse’s role in public
health. Students will learn the value of teamwork and why it is important in when
working in health care and disease prevention.
A5. Number of Items 10 Questions
At the end of the course, students will be required to complete a case study with open
ended questions. This will help students to reflect on the course concepts listed within
the modules and indicate to the instructor that the student has retained the information
, taught within the course. Students' answers will be evaluated and graded by the
instructor and grades will be posted as soon as all assessments are completed.
