◉ Registering with the board & current information
Answer: Each licensee must provide in writing the addresses of all
permanent and extended temporary locations in which the licensee
practices and the licensee's office telephone numbers, mailing
address, and residence address. Any notice required to be given by
the board to a licensee shall be sent by mail to the licensee's most
current mailing address shown on records of the board. The board
may attempt to give or serve notice at any other address or location
on file with the board if service to the mailing address is
unsuccessful. Service of a required notice is complete on the date of
mailing to the licensee's mailing address.
