,COMMUNITY ASSESSMENT AND EVALUATION

Community refers to the dimensions of place, people/person, & function. In a community
assessment the nurse looks at data for the dimensions of place, people/person, and
function. When measuring the dimension of people, the community health nurse would
access data sources from the census data, tourist bureau, civic groups, and churches.
Libraries, police stations, welfare agencies, and state departments would be data sources to
measure the function's dimension. The dimension of place would be measured with data
sources of maps, newspapers, and the local housing office.

When completing a community assessment, the nurse will include community system
categories found within a community, which include politics, education, recreation,
economics, and religion.

Active vs. Passive Partnerships-Community partnerships focus on active participation by
community leaders, businesses, and organizations to develop programs with healthcare
professionals. These programs have a larger goal of collaboration to achieve an outcome
that benefits community members.

Example of an active partnership-A business that provides low-cost snacks to an after-school
program is an example of a community partnership.

Example of an passive partnership A city creating a new park; a hospital providing a
vaccination clinic; a recreational center offering cooking classes.

During a community assessment, the nurse will perform data gathering and data generation.
Data generation is a process of identifying data that does not exist but is developed through
interactions with the community. Examples of data generation include information about the
community's values, beliefs, and social norms. Data gathering is the process of collecting
information that exists, such as the age of residents, racial distribution, and health
organizations. To prepare for a Grant Proposal the nurse identifies resource eligibility with a
needs assessment.

A windshield survey is a method to collect data about the community by observing the
community through a car or riding in public transportation. To conduct the windshield survey,
the nurse will need to understand the community's boundaries to ensure that the community
is adequately observed. The population size, socioeconomic qualities, and governmental
structures are observed during the survey.
