ISSUES IN THE HYBRID RECORD
Upon reading the given scenario of keeping hybrid records at Anywhere Memorial
Hospital, there are issues with this type of clinical documentation.
First, the hybrid system required a full-time HIM employee to gather and scan all paper
records into the system. If the scanning system goes down, complete records will not be able to
be accessed in the paper or digital records because of backlog issues. Then due to the system
being down, the HIM staff would have to find the paper documentation and take it back to the
patient care unit. Which in turn would result in lost documents and misplaced patient records.
The increased need for more HIM personnel needed to locate and scan records may drain the
resources and money of the hospital.
Second, providers will not always have complete access to patient records if they are in
paper form because paper records could still be in queue for scanning or other personnel might
have a part of that patients’ record. Which can cause a delay in diagnosis and treatment since
providers will not have full access to that patient’s chart. The delay in diagnosis and treatment
can lead to billing issues as charges per visit will be postponed causing deficits in the revenue
cycle.
FULLY DIGITAL AND HYBRID RECORDS
A fully digital recording system will address and solve the issues caused by the hybrid
recording system because it will remove the need for paper charting, scanning of documents,
uploading, and the human errors of misplacing or having the records in a different location. All
hospital staff will have access to patient charts, allowing for real time documentation of patient
