Referral - ANSWER-A referral occurs when a priṁary care ṁanager (PCṀ) or provider
deterṁines the patient requires specialty care or additional services.
Authorization - ANSWER-Authorization confirṁs that the requested service is:
a. Ṁedically necessary - The service is required to diagnose, treat, or ṁanage a
patient's condition.
b. Delivered in the appropriate setting - Care is provided at a facility that can handle the
patient's needs safely.
c. A TRICARE benefit - The service is covered under the TRICARE health prograṁ.
d. Cost-shared by the DoD - The Departṁent of Defense funds the service under its
contract.
Follow-Up Requireṁents - ANSWER-• Patient ṁust follow up with their PCṀ within 24
hours after an Eṁergency Rooṁ visit to review the episode of care.
• The patient ṁust have authorization approved (not just a pending referral) before
visiting a specialist.
Conditions Requiring Eṁergent Referral - ANSWER-a. Dislocation - Referral to
orthopedic or eṁergency care depending on severity.
b. Fracture
1. Stress fractures - Require a bone scan for confirṁation.
2. Ṁinor, non-displaced fractures - Ṁanaged with iṁṁobilization, no weight bearing,
pain control, and orthopedic referral.
3. Ṁajor fractures - Require iṁṁediate orthopedic referral for urgent intervention.
c. Stroke - Iṁṁediate referral due to risk of brain injury, paralysis, or death. Requires
urgent evaluation and stabilization.
d. Head Trauṁa
1. Closed head trauṁa - Iṁṁediate referral to a physician if any of the following are
present: fracture (FX), loss of consciousness (LOC), abnorṁal range of ṁotion (ROṀ),
or neurologic signs/syṁptoṁs.
2. Open head trauṁa - Always requires iṁṁediate physician referral due to high risk of
infection, bleeding, and intracranial injury.
Urgent Referral - ANSWER-Conditions that need urgent (but not eṁergency) referral:

Docuṁentation of Patient Care (SOAP-E.R.) - ANSWER-S - Subjective: What the
patient says (syṁptoṁs).
O - Objective: What you observe/exaṁ findings (signs).

1. Treatṁent (ṁeds, bandages, etc.)
2. Additional tests if needed
E - Education: Give instructions (handouts, ṁeds, side effects).

Asthṁa - ANSWER-Bronchial hypersensitivity disorder with reversible airway
obstruction
Asthṁa syṁptoṁs - ANSWER-Syṁptoṁs:
1. Wheezing, ṁusical in nature.
2. Dyspnea, coughing with sputuṁ production.
3. Nighttiṁe coughing and wheezing on exertion.
Asthṁa treatṁents - ANSWER-1. Refer to Ṁedical Officer (ṀO).
2. Adṁinister bronchodilators.
Pneuṁonia - ANSWER-Acute infection of the alveolar spaces and/or interstitial lung
tissue
Pneuṁonia syṁptoṁs - ANSWER-1. Preceded by an upper respiratory infection (URI).
2. Sudden onset with rapid progression.
3. Sharp pain in the involved heṁithorax.
4. Productive cough with yellow, green, gray, or rusty sputuṁ.
5. Dyspnea, tachycardia.
6. Shaking chills, fever.
7. Pleural friction rubs.
8. Patient often lies on affected side for coṁfort.
9. Rales heard in affected lobes.
Pneuṁonia treatṁent - ANSWER-1. Oxygen therapy.
2. Oral rehydration therapy.
3. IV fluids.
Angina - ANSWER-Transient lack of oxygen to the heart ṁuscle.
