Therapeutic Drug Monitoring Process of analyzing fluids to determine
concentration of a prescribed drug present at a
particular time along with correlation of the drug
with the effect on the patient. It is performed to
measure compliance, to establish a treatment
regimen, and avoid toxic effects.

Peaks and Troughs -Peak: highest concentration, measure 1 hour after
infusion of drug.
-Trough: lowest level, measure 30 minutes prior to
next dose.

Drug Absorption -Three ways to administer: orally, rectally,
parenterally (IM or IV).
-Oral: absorbed by GI tract, go to liver through
hepatic portal vein then circulation "first pass
elimination".
-Parenterally: drugs that can't withstand the
condition of GI tract or are altered through first
pass.

, Drug Distribution -Way the drug is carried: ionic or free form, by
carrier molecule.
-When it reaches the site: must get inside cell, free
drugs pass easily through cells, protein bound
drugs are inactive because they can't pass through.

Drug Metabolism -Major site of elimination is the liver, tries to
convert drug to water soluble compound to
excrete through kidney.
-Other elimination sites are kidneys, lungs, and GI
tract.

Drug metabolism kinetics -First order Kinetics: rate of metabolism is
dependent on the concentration of drug. Higher
concentration = faster metabolism.
-Zero order kinetics: constant amount of drug
metabolized over time regardless of concentration.
-Half life: period of time in which 1/2 of the drug
decays.

Drug Excretion -If it can become water soluble, kidneys will
excrete it.
-Pharmokinetics: way to represent the deposition
of a drug over time in order to better interpret
levels so that the dosage can be adjusted.

Factors that affect drug disposition -Age: neonates little capacity, kids 6-10 double the
adult dosage, adults rate slows, elders poor rate of
disposition.
-Pregnancy: many factors increased plasma
volume and change in protein concentration.
-Diseases like liver or renal disorders.

Drug specimen collection Blood collected in either red top or heparin tubes
