Right documentation ALWAYS DOCUMENT EVERYTHING! Document
administration only after the med has been given,
including time, route, vital signs, etc.


Right reason or indication Addresses the appropriateness in use of the
medication to the patient. Confirm the rationale for
giving the med by researching the patient's medical
history.


Right response Refers to the drug and its desired response in the
patient. Continually assess the achievement of the
desired response (vital signs, weight, edema, I/O,
lab values, etc.).


Right to refuse Patients ALWAYS have the right to refuse! DO NOT
FORCE! Document the refusal and the reason why.
