Abstract
This review paper synthesizes current evidence-based principles governing
antepartum care and fetal assessment in the context of Bachelor of Science in
Nursing (BSN) education. Drawing from the seventh edition of Perry's Maternal
Child Nursing Care, the fourth edition of the AWHONN Core Curriculum, and
2026 ACOG Practice Bulletins, this paper addresses five core competency
domains: maternal physiological adaptations to pregnancy, fetal development
milestones and well-being assessment, common pregnancy complications
requiring nursing surveillance, prenatal screening and diagnostic modalities,
and patient education strategies grounded in the NCSBN Clinical Judgment
Model. Emphasis is placed on distinguishing physiologic from pathologic
findings, recognizing emergent conditions such as preeclampsia with severe
features and ectopic pregnancy, and applying evidence-based nursing
interventions that optimize maternal and fetal outcomes across the antepartum
period.

1. Maternal Physiological Adaptations to Pregnancy
Pregnancy induces profound, systemic physiological changes that are essential for
supporting fetal development and preparing the maternal body for parturition. Understanding these
adaptations is fundamental to accurate clinical assessment, as failure to distinguish normal from
abnormal findings may lead to either unnecessary intervention or delayed recognition of serious
complications. The cardiovascular system undergoes some of the most significant alterations: blood
volume increases by 30-50% by the third trimester, cardiac output rises proportionally, and
hemodilution produces a physiologic anemia with hemoglobin values typically ranging from 11 to
12 g/dL [1]. Nurses must differentiate this expected dilutional anemia from true iron-deficiency
anemia, which requires supplemental iron therapy. Additionally, the supine hypotensive syndrome,
caused by compression of the inferior vena cava by the gravid uterus after 20 weeks, necessitates
patient education regarding lateral recumbent positioning during rest.
Respiratory adaptations include a 30-40% increase in tidal volume and a proportional
decrease in functional residual capacity, driven primarily by progesterone-mediated stimulation of
the respiratory center and upward displacement of the diaphragm by the enlarging uterus. While
mild dyspnea on exertion is common and physiologic, nurses must remain vigilant for signs of
pathologic respiratory distress such as dyspnea at rest, oxygen saturation below 95%, or chest pain
suggestive of pulmonary embolism. Renal adaptations include a 50% increase in glomerular
filtration rate and associated creatinine clearance elevation, which results in physiologic glycosuria
that must be distinguished from the glycosuria of gestational diabetes. Trace glycosuria may be a
normal finding, whereas proteinuria of 1+ or greater warrants thorough evaluation for preeclampsia
[2]. Gastrointestinal changes including decreased motility and lower esophageal sphincter tone
predispose patients to constipation and gastroesophageal reflux; dietary counseling regarding fiber
intake, hydration, and small frequent meals constitutes a key nursing intervention.
2. Fetal Development and Assessment
Systematic fetal assessment integrates gestational age-specific developmental milestones
with clinical monitoring techniques to evaluate fetal well-being throughout the antepartum period.
Between 8 and 12 weeks, the fetal heart rate becomes detectable by Doppler ultrasonography with a
normal range of 110-160 beats per minute, and organogenesis is largely complete, making this the
critical window for teratogen avoidance counseling. Naegele's rule (LMP minus 3 months plus 7
days plus 1 year) remains the standard clinical method for estimating the expected date of delivery.
Between 16 and 20 weeks, the primigravida patient typically begins to perceive fetal movement
(quickening), whereas multigravida patients may detect movement as early as 18 weeks. This
period also represents the optimal window for the detailed anatomy scan, which assesses fetal
structures for congenital anomalies [3].
Viability, generally defined as 24 weeks of gestation, marks a pivotal clinical threshold. At
this stage, surfactant production begins in the fetal lungs, and the glucose challenge test for
gestational diabetes mellitus is administered between 24 and 28 weeks. Rh-negative patients require
