The Human Microbiome
• The human microbiome is defined as the sum total of all microbes found on
and within a normal human.
• Mammals have evolved their form and physiology in close association with
their resident microbes, highlighting the microbiome's critical importance to
host health and function.

Colonization, Infection, and Disease
• Colonization: For the most part, resident microbiota live on and in the host
without causing disease.
• Infection: Occurs when microbes overcome host defenses, enter tissues, and
multiply.
• Disease: A pathologic state resulting from the cumulative effects of infection,
where tissues and organs are damaged or disrupted.
• Infectious Disease: A disease state caused directly by microorganisms or their
toxic products.

The Human Microbiome Project (HMP)
• Launched in 2008, the HMP is a global research initiative.
• Its goals are to characterize the microbes present in healthy human bodies
and to understand how the microbiome differs in various diseases.
• The project utilizes advanced genome sequencing and "big data" analysis
tools.

Key Findings of the Human Microbiome Project
• Genetic Diversity: While human cells possess about 21,000 protein-encoding
genes, the microbiota contribute approximately 8 million.
• Unexpected Locations: Microbes have been found in numerous locations
previously considered sterile.
• Viral Load: Human feces can contain as many as 100 million viruses per gram.
• Pathogen Presence: All healthy individuals appear to harbor potentially
dangerous pathogens in low numbers.
• Health Influence: The composition of the intestinal microbiota can
significantly impact overall host health.

Factors Weakening Host Defenses
Host susceptibility to infection is increased by various factors, including:

• Age: Old age and extreme youth (infancy, prematurity).

• Medical Interventions: Surgery and organ transplants.

Colonization of the Fetus
• Previously, it was believed that the uterus and fetus were sterile during
development.
• However, analysis of newborns' stools collected before their first feeding
indicates colonization by diverse bacteria in utero.

Colonization of the Newborn
• Vaginal Birth: The mother's vaginal microbiota is a significant source of
colonization for newborns during passage.
o Lactobacillus species provide enzymes essential for digesting milk.
o Other microbial species help protect the baby from skin disorders and
other conditions.
• Human Milk: Contains approximately 600 bacterial species and unique sugars
that the infant cannot digest but that nourish beneficial gut bacteria.

Sources of a Baby's Microbiome
Pathogenicity and Opportunistic Pathogens
• Pathogen: A microbe whose parasitic relationship with its host results in
infection and disease.
• Pathogenicity: The inherent potential of a microorganism to cause disease.
o True Pathogens: Capable of causing disease in healthy individuals with
intact immune defenses.
o Opportunistic Pathogens: Cause disease only when host defenses are
compromised or when they colonize a site not natural to them.

Virulence
• Virulence: Refers to the relative severity of a disease caused by a specific
microbe, it's the degree of pathogenicity.
• A microbe's virulence is determined by its ability to:
o Establish itself within the host.
o Cause damage to the host.
