•Emergency severity index (ESI) 5-level ED triage system (Level 1 = most urgent, Level 5 = least urgent)
Uses a triage algorithm to assign level
Based on:
Acuity (how sick the patient is)
Expected resource use (labs, imaging, IV meds, etc.)
Determines who gets treated first
Supported by: Emergency Nurses Association (ENA) & American College of
Emergency Physicians (ACEP)
SUPER HIGH-YIELD ADD-ON (you NEED this for exams)
ESI 1: Immediate life-saving intervention needed (e.g., cardiac arrest, apnea)
ESI 2: High risk / confused / severe pain or distress
ESI 3: Stable BUT needs multiple resources
ESI 4: Stable, needs 1 resource
ESI 5: Stable, needs NO resources

KEY TEST PEARL (don't miss this):
If patient needs immediate life-saving intervention → ESI 1 (no questions
asked) © If not, then decide based on risk + resources
ESI on exam*

ESI-1 •ESI-1: assess for threats to life
•Is the patient in imminent danger of dying?
Stability of vital functions (ABCs): unstable
Life threat or organ threat: Obvious
Immediate life threat → treat NOW
Ask: "Is the patient about to die?"
ABCs unstable (airway, breathing, circulation)
Requires immediate life-saving intervention
Obvious life or organ threat

ULTRA HIGH-YIELD PEARL:
If you even think they need resuscitation → ESI 1
Examples (know these):
Cardiac arrest
Respiratory failure/apnea
Unresponsive patient
Severe shock
