SECTION 1: STUDY GUIDE
========================================
1.1 The Nursing Process (ADPIE)

The nursing process is a systematic, patient-centered, outcome-oriented
method of delivering nursing care. It is the foundation of clinical
decision-making and is tested heavily on NCLEX.

ADPIE Mnemonic

| Letter | Phase | Description | Key Questions |
|--------|-------|-------------|---------------|
| A | Assessment | Collect subjective and objective data | What does the
patient say? What do I see, hear, smell, feel? |
| D| Diagnosis | Analyze data to identify nursing diagnoses (actual or
risk) | What is the problem? What is the etiology? What are the defining
characteristics? |

,
|P | Planning | Set measurable, realistic patient goals; prioritize
interventions | What outcomes do we want? What are the SMART goals?
|
| I| Implementation | Perform nursing interventions (independent,
dependent, collaborative) | What actions will I take? Am I using
evidence-based practice? |
| E | Evaluation | Determine if goals were met; revise plan as needed |
Did the patient achieve the outcome? What needs to change? |

Types of Nursing Diagnoses (NANDA-I)

- Actual diagnosis: Patient currently experiencing the problem (e.g.,
Acute Pain, Impaired Gas Exchange)
- Risk diagnosis: Patient at risk for developing a problem (e.g., Risk for
Falls, Risk for Infection)
- Health promotion diagnosis: Patient desires higher level of wellness
(e.g., Readiness for Enhanced Nutrition)
- Syndrome diagnosis: Cluster of diagnoses that occur together (e.g.,
Post-Trauma Syndrome)

SMART Goals

- Specific – Clearly defined, focused on patient behavior
- Measurable – Quantifiable (vital signs, pain scale, lab values)
- Achievable – Realistic given patient condition
- Relevant – Meaningful to patient and outcomes

,
- Time-limited – Specific time frame for evaluation

Example: "Patient will verbalize pain level less than 3/10 within 30
minutes of medication administration."

1.2 Clinical Judgment Measurement Model (NCLEX Next Gen)

The NCSBN Clinical Judgment Measurement Model (CJMM) is the
framework for Next Generation NCLEX (NGN). It includes six
cognitive skills:

| Layer | Skill | Description | Example Question Type |
|-------|-------|-------------|----------------------|
| 1 | Recognize Cues | Identify relevant patient data (subjective,
objective, lab, imaging) | Highlight text, matrix |
| 2 | Analyze Cues | Link cues to clinical conditions; identify concerning
findings | Matrix, drag-and-drop |
| 3 | Prioritize Hypotheses | Rank problems by urgency (ABCs, Maslow,
safety) | Ordering, ranking |
| 4 | Generate Solutions | Identify appropriate interventions | Multiple
response, matrix |
| 5 | Take Actions | Implement nursing interventions | Matrix, drop-down
|
| 6 | Evaluate Outcomes | Assess if interventions were effective | Matrix,
multiple choice |
