Opening Questions
How did the simulated experience of Carla Hernandez’s case make you feel?
The experience made me feel a sense of urgency. It was the first VSim to have a patient in active labor and knowing that the issue would be a compressed umbilical cord certainly added urgency. Describe the actions you felt went well in this scenario.
I was able to calmly and efficiently read the provider orders and have a general understanding of what I needed to do. Scenario Analysis Questions1
EBPConsider the emergent nature of umbilical cord prolapse. What nursing actions should be initiated immediately? Why?
The initial and simultaneous nursing actions should be to call for help, do not leave the patient, inform the patient of what is happening, assist with measures to relieve pressure of the cord on the fetus. Also, position patient in Trendelenburg in order to relieve cord pressure and follow provider orders to administer fluids and medications. The initial nurse action should be to relieve the cord compression and increasing fetal oxygenation. PCCWhat is the priority need for Carla Hernandez at this time?
The priority need other than repositioning, relieving pressure and ensuring safety for her baby would be to support and calm Carla. Providing comfort that the healthcare team is going to do what they need to in order to keep her and the baby safe. Also, explanation/education of what
is happening. T&CWhat key elements would you include in the handoff report for this patient? Consider the situation-background-assessment-recommendation (SBAR) format.
