7.1 Defining Medication Errors
● Medication Errors - any preventable event that may cause or lead to
inappropriate medication use or patient harm while the medication is in the
control of the health care professional, patient, or consumer. Such events may be
related to professional practice, health care products, procedures, and systems,
including prescribing, order communication, product labeling, packaging, and
nomenclature, compounding, dispensing, distribution, administration, education,
monitoring, and use. (NCC MERP, 2018b)
● Medication Error Index - conceptual framework that places medication errors
into nine categories based on the extent of harm an error can cause
○ Category A is a medication error in which no harm occurred to the patient
○ Category H is a medication error that resulted in permanent harm.
