 CARBAMAZEPINE (Tegretol): Neural tube defect
 LITHIUM (Eskalith): Ebstein anomaly (avoid in pregnancy, especially 1st
trimester)
 DIVALPROEX SODIUM (Depakote): Neural tube defects, specifically spina bifida

PB 181 – SIDE EFFECTS OF MOOD STABILIZERS
 LAMICTAL: SJS (severe rash)
o S/S OF SJS:
 Fever *
 Sore throat
 Facial swelling
 Tongue swelling
 Rash
 Skin sloughing
 Prodromal headache, malaise, arthralgia, painful mucous membranes
may occur before rash occurs.

PB 182 – CARBAMAZEPINE
o Black box warning: Agranulocytosis (decreased WBCs) and aplastic
anemia (pallor, fatigue, headache, fever, nosebleeds, bleeding gums,
skin rash, SOB)
o SJS, particularly in Asians (screen for HLAB-1502 allele before initiating)
 HLAB-1502 allele is highly associated with outcome of
carbamazepine-induced SJS
 Check HCG before starting in patient of child-bearing age (12-51y/o)
on mood stabilizer
FOLIC ACID – supports neural tube development during the first month that a
woman is pregnant
o It is recommended that all women planning or capable of becoming
pregnant take 0.4 - 0.8mg of folic acid daily
