PHASE 1
1a. Write a paragraph in which you describe population health and the role that Healthy People
2030 Social Determinants of Health play in guiding efforts toward improving population health.
Population health focuses on positive health outcomes for varying groups and the impact
external factors have on these outcomes. Its goal is to provide interventions to keep people
healthy rather than those for sick care. These outcomes are often affected by social determinants
of health (SDOH). SDOHs are conditions in the environment where individuals or groups live
that drive population health. These include factors such as socioeconomic status, education, and
access to healthcare and food. By identifying these determinants, interventions can be developed
to better suit the needs of a specific population and their healthcare needs can be better met.
Healthy People 2030 provides a framework to use the interventions in creating and promoting
overall population health and, at the same time, reducing health disparities and inequity.
1b. Write a paragraph summarizing the difference in the role of the entry level nurse and the
advanced professional nurse in advocating for population health.
Based upon AACN Advanced-Level Domain 3 competencies, the role of the entry-level
nurses in population health advocacy focuses more on direct patient care and identifying
immediate needs of individuals on the local level. This can be in the form of promoting
preventative care or patient education. On the other hand, advanced professional nurses are more
often found in leadership roles that involve analyzing population health trends, implementing
public health initiatives, and influencing health policy. This can provide improvements on health
outcomes that lead to systemic, national, or global changes.
1c. Using data and resources from the websites above or other credible sources, write a
paragraph in which you discuss several health issues or concerns in your chosen U.S. city,
county, or state. Include data that supports your assessment.
Detroit, Michigan has an above average of adults reporting high blood pressure, diabetes
and obesity. In 2022, it was reported that 46.1% of adults have high blood pressure. The national
average was 31.2%. Compared to the national average of 32.5%, Detroit reports 44.8% of the
adults are obese. Diabetes is reported at 17.7% in Detroit which is above the national average of
11.4%. According to the National Institute of Health (NIH), Many cities that report high rates of
obesity often report high blood pressure and diabetes as being comorbidities. Most of these
diagnoses are preventable diseases and influenced by lifestyle factors.
1d. Write a paragraph discussing the role the advanced professional nurse can play in advancing
health equity.
, According to the Robert Wood Johnson Foundation (RWJF), everybody is entitled to
quality healthcare regardless of their background or circumstances. Advanced professional
nurses can use their clinical knowledge and expertise and leadership skills to advocate equity in
healthcare. They can lead initiatives that promote culturally competent care, implement
evidence-based interventions tailored to underserved populations, and influence policies that
prioritize equitable resource distribution. They can participate in community outreach, mentor
novice nurses, and engage in interdisciplinary collaboration to help reshape healthcare systems to
be more effective for all individuals.
1e. To empower yourself as a health policy influencer it is important to effectively communicate
with public leaders. Create a contact list of at least one public leader for each of the following
governmental levels. Include name, title, and contact information.
