Phase 1: Include the following deliverables in your e-portfolio:
1a. A paragraph that describes population health and the role that Healthy People 2030
Social Determinants of Health play in guiding efforts toward improving population
health.
Population health focuses on improving the health outcomes of groups by addressing the
factors that influence their well-being, such as access to care, social environments, and
economic conditions. Healthy People 2030 identifies Social Determinants of Health (SDOH)
as critical drivers of health disparities and offers achievable methods for tackling these
issues. The five SDOH domains—economic stability, education access and quality, health
care access and quality, neighborhood and built environment, and social and community
context—serve as a guide for addressing disparities that damage overall population health.
Efforts like improving safe housing and expanding access to nutritious food directly connect
with these domains to reduce preventable diseases and improve health equity (Healthy
People 2030, n.d.). For example, the text by Dawes and Gonzalez highlights how political
determinants like policy decisions significantly influence these social factors, shaping access
and outcomes at a population level. Together, these insights emphasize the importance of
addressing upstream social and political drivers to improve health across diverse
communities.

1b. A paragraph summarizing the difference in the role of the entry level nurse and the
advanced professional nurse in advocating for population health
An advanced practice nurse (APN) advocates for population health by using their expanded
expertise to influence systemic change. In contrast, an entry-level nurse operates more
directly with individuals and smaller groups. Entry-level nurses prioritize focusing on
recognizing health trends, educating patients, and identifying community health issues
through direct care. APNs, on the other hand, evaluate large-scale data, design
interventions that address root causes of health disparities, and influence policies that
impact broader populations. They lead multidisciplinary efforts, collaborate with
policymakers, and secure resources to improve health equity. Unlike entry-level nurses,
APNs have the authority and training to implement and drive healthcare reforms that
tackle structural barriers to health, shaping outcomes on a regional or even national scale
(American Association of Colleges of Nursing).

1c. A paragraph that discusses several health issues, concerns, or behaviors in your city,
county, or state and includes supporting data
Las Vegas and Clark County, Nevada, face significant health challenges compared to
national averages, particularly due to a shortage of physicians. With 1,830 residents per
primary care physician, compared to the national average of 1,330, access to care is
limited, leading to longer appointment wait times and reduced availability of
preventative services (County Health Rankings & Roadmaps, 2024). This gap in care
