NUR 242 Week 1 Practice Questions 2025 Medical-
Surgical Nursing Concepts (Galen College of
Nursing)
1
Dehiscence: Partial of complete separation of the tissue layers during the healing process

Serous: Clear, watery fluid from plasma

Debridement: Removal of necrotic tissue

Eschar: Necrotic tissue

Approximated: Brought together

Serosanguineous: Drainage is pink to pale red and contains a mix of clear fluid and red, bloody
fluid

Evisceration: Total separation of the tissue layers, allowing the protrusion of visceral organs
through the incision

Fistula: Abnormal connection between two internal organs or between an internal organ and the
outside of the body

Shear: Phenomenon that occurs through the relationship between friction and gravity

Sanguineous: Usually indicates bleeding and is bright red

Question 2

This is an example of a wound with eschar or necrotic tissue.

Debridement of wounds may be completed in four different ways. In sharp, a scalpel or scissors
is used to separate and remove dead tissue. Special training is required for this sharp debridement
and must be performed by a wound ostomy continence nurse. Mechanical is performed using a
scrubbing force or damp-to-damp dressings. Agents such as collagenase enzymes are most
recommended in chemical debridement. Hydrocolloid and clear absorbent acrylic dressings trap
wound drainage against eschar, causing the body's own enzymes to break down necrotic tissue in
autolytic debridement. The most selective debridement is autolytic and it takes longer than the
rest but is the most preferred method causing the least damage to healthy surrounding and
healing tissues.

Question 3
A Penrose drain has an open end that drains onto a dressing.

TRUE: The Penrose drain is not considered a closed-wound drain. Penrose drains are made of
latex and drain by gravity. It is always important to verify latex allergies for any client.

Question 4
Maintaining the wound suction of a drain hastens the healing process by draining excess exudate
that might otherwise interfere with the formation of granulation tissue.

TRUE

Question 5
What type of drain is this?
