Physical Restraints:
• Soft wrist restraints
• Mitt restraints
• Vest restraint
• Limb restraints
Criteria for use
• Patient is an immediate danger to self or others.
• Less restrictive interventions have failed.
• Used to prevent interruption of life-saving treatment (removing IV, ET tube).
• Requires a healthcare provider order.
• Time-limited and reassessed frequently.
Nursing Care
• Assess circulation every 2 hours or per policy.
• Remove restraints regularly for ROM, hygiene, nutrition, toileting.
• Document behavior requiring restraint.
• Continue attempting least restrictive alternatives.
Chemical Restraints:
• Haloperidol
• Lorazepam
• Olanzapine
Criteria
• Severe agitation
• Violent behavior
• Patient poses immediate danger
• Used only when therapeutic communication and de-escalation fail
Seclusion
Criteria
• Violent or aggressive behavior
• Danger to others
• Last resort after less restrictive measures fail
