• Ask the patient to state his or her full
name and birth date, and compare these
with the patient’s identification (ID) band
and the medication administration record
(MAR).
• Many facilities have electronic health
records (EHRs) that allow the nurse to
directly scan the bar code from the
patient’s ID band.
