2. History of Present Illness (HPI)
James Taylor is a 39-year-old male presenting with progressively worsening shortness of breath for the
past 3 days. Initially experienced dyspnea during work activities; now short of breath even at rest.
He reports:
Increasing cough, initially dry, now productive with yellow sputum.
Fever peaking at 38.7°C (101.6°F).
Pleuritic chest pain on the right—sharp, worsens on inspiration.
Orthopnea requiring 2 pillows last night.
Fatigue and reduced exercise tolerance.
No hemoptysis, no leg swelling, no recent immobilization.
