thorough, creates a baseline/platform and develops dr-pt relationship. Also called complete,
expanded or new patient.



Focused History - ANS -One of two basic types. Used for an established patient or in a
hospital/urgent care setting. Exam is restricted to pertinent body system, and focused on CC.
Also known as problem oriented or basic.



Inventory History - ANS -Alternative to the comprehensive history that is not as detailed and a
little quicker.



Interim History - ANS -Alternative to the focused history, is for an established pt that has not
been seen in awhile. It attempts to chronicle the events since the last visit.

Objective Information - ANS -This is what you find in the physical exam.



Four C's to establish effective communication - ANS -Courtesy, comfort, connection,
confirmation



How to establish effective communication - ANS -Use the Four C's, be mindful of question types,
use different interview techniques, and utilize tools such as Ask Me 3, and teach back method.



Different Question Types - ANS -Open-ended, direct/closed, graded response, leading and
multiple



Open-ended Question - ANS -Best when seeking general information, often used by beginner
interviewers.



Direct/Closed Questions - ANS -Best when seeking specific facts. Help outline CC and PMHx,
answered in a brief sentence.



Graded Response - ANS -Qualifying symptoms with a number scale or figure or some other
measure.



Ask Me 3 - ANS -Method for assessing pt/dr communication. Pt should ask (or be able to
answer): What is my main problem? What do I need to do? and Why is it important for me to do
this?
