Central Line Dressing Change Procedure
Preparation
• Remove obstacles: Roll up sleeves, secure lanyard/badge behind you to avoid breaking the sterile
field.
• Masks:
• Nurse wears a mask.
• Patient wears a mask to prevent contamination of the site.
• Positioning:
• Have the patient turn their head away from the central line site.


Removing Old Dressing
1. Hand hygiene → apply clean gloves.
2. Stabilize the line:
• Hold the catheter with one hand while removing dressing to prevent pulling.
3. Remove tegaderm:
• Pull dressing outward, away from the site (reduces skin trauma).
4. Inspect the site:
• Look for redness, swelling, drainage, or signs of infection.
5. Remove gloves after inspection.


Setting Up Sterile Field
• Open sterile kit:
• Pull first flap away from you.
• Only touch the 1-inch border of the drape.
• Remove sterile gloves from kit and place them on the edge of the sterile field.
• Don sterile gloves without contaminating them.


Sterile Supplies Inside Kit
• 3 alcohol swabs
• Sterile 4x4 gauze
• Tape strip
• Tegaderm (may have CHG gel center)
• Chloraprep applicator


Site Cleaning
6. Alcohol swabs:
• Scrub insertion site and catheter lumens.
