CHARACTERISTICS

Paper records are physical records that are kept at one location. They need physical space

for storage. Their accessibility is limited because they can only be in one place at a time. They

also take time to find. Information is amended typically by drawing a single line through the

original text and then dating and signing it if correcting. If adding something, then you would

add what you need and then date and sign (Triplett, 2024).

Hybrid records are a mixture of paper records and electronic records. Storage for hybrid

records is both physical and digital storage space. Accessibility is better than paper records

because some information is available in more than one place and is easier to access. Amending

a hybrid record would be the same as physical records. For electronic records, you would clearly

label the updated information, and make sure there is a way to see the original content, modified

content and who made the change (Triplett, 2024).

Fully electronic health records utilize digital storage space. This is an advantage because

you can scale how much storage is needed. It is almost finite. Accessibility of fully electronic

health records is the best of the three because all information should be stored digitally, where

access is easy and faster. How information is amended will vary from place to place. The

original record should never be erased, and changes should be labeled conspicuously. When a

change is made, it should show the person who made the change and the date.
