, LES 1: HOMEOSTATIC REGULATION - GEZONDHEIDSPSYCHOLOGIE 2
• Homeostatische regulatie = innerlijke balans stabiel houden ondanks veranderingen in de externe omgeving.
• Central nervous system = interface for interaction with external environment.
• Stress = threat to homeostasis --> stressor (fysiek; bottom up, en psychologisch; top down).
--> compensatory stress response.

Feedback controle: opnieuw in balans krijgen
– Temperatuur: via hypothalamus.
o T° stijgt -> ZS geeft signaal; bloedvaten zetten uit en zweetklieren actief -> lichaamswarmte wordt afgegeven.
o T° daalt -> ZS geeft signaal; bloedvaten krimpen en zweetklieren inactief, indien T° blijft dalen gaan spieren
onvrijwillig samentrekken waarmee ze warmte genereren -> lichaamswarmte wordt bewaard.

– Blood pressure: Baroreceptor reflex via medulla in hersenstam.
o Baroreceptor detecteert veranderingen in arteriële druk -> signaleert medulla -> hartslag wordt aangepast.

– Blood pH level/arterial carbon dioxide pressure (PaCO2)
o Hoge pH -> te veel O2 -> minder respiratoire functie -> normaal pH.
o Lage pH -> te weinig O2 en te veel CO2 -> meer respiratoire functie -> normale pH.

Feedforward controle: anticiperen
Anticiperen op perturbations (verstoringen) voor ze zich voordoen. Vaak geleerd door klassieke conditionering.
- Vb ‘excercise hyperpnea’: meer ventilatie en harstlag voor de start van fysieke inspanning én voor verhoging PaCO2.

1. Hiërarchie van homeostatische controle
1.1 Intrinsieke controlemechanismen (orgaan niveau)
Organen passen hun functioneren aan als reactie op trage lokale veranderingen. Kunnen
op zichzelf functioneren: vb: frank sterling mechanisme (hart) =
• Indien meer bloed terugkomt -> kamer en ventrikels meer gevuld waardoor wanden
meer gestretcht -> meer spanning op spiervezels -> sterkere contractie van het hart.
• Hartrespons op flow demand door de systemische circulatie.

Voorwaarde: condities zijn relatief stabiel. Indien omgeving verandert, is dit niet meer
mogelijk en moet men aanpassen en bijsturen.

1.2 Autonome controlemechanismen (Autonomisch zenuwstelsel ANS)
– Viscerale organen hebben gelimiteerde awareness en vrijwillige controle. Deze
processen gebeuren automatisch, op lager niveau in de hersenstam.
– Centraal controlemechanisme: negatieve feedback.
– ANS bestaat uit:
o Afferent: sensorische pathway (bottom-up).
o Efferent: motorische pathway (top-down).
o Wederkerige regulatie van orgaanfunctie.
o Divisies: sympathisch (SNS), parasympathisch (PNS) en enteric. Elke divisie heeft een sensorische pathway van
de organen via de ganglia naar hersenstam. Elke divise bestaat uit 4 componenten:
§ Descending autonomic en pre-ganglionische fibers (van hypothalamus -> spinal cord).
§ Ganglion (deel van lokale regulatie reflexen).
§ Postganglionic fibers (meer uitgewerkte berichten).
§ Neuroeffector junctions (op target tissue, zenuw impuls -> motor action).

Sympathische divisie (ANS)
• Meer actief tijdens stress, cruciaal voor fight/flight respons.
• Ratio 1 pre- vs 10 postganglionic nerves (1:10) waardoor algemeen, brede invloed op viscera.
® Dicht bij elkaar geïntegreerde acties over verschillende organen heen (in sympathie met elkaar).

• Neurotransmitters =
– Acetylcholine (preganglionic).

, – Norepinephrine (postganglionic): smooth muscle cells, cardiac muscles and pace maker: activating function.
• Except: (a) sympathetic pre-ganglionic nerves release acetylcholine at adrenal medulla -> release of
catecholamines (Nor-/Epinephrine) into blood.
• Except: (b) sympathetic nerves release acetylcholine at sweat glands (hands, feet).

Parasympatisch (vegal) divisie (ANS)
• Meer actief tijdens rust: ondersteunt energiebewaring, reproductie en digestie.
• De ganglia zijn meer specifiek en dichter bij target organ.
• 1:3 pre- vs postganglionic nerves waardoor lokaal, specific actions directed at one organ.
• Neurotransmissitters =
– Acetylcholine (preganglionic).
– Acetylcholine (postganglionic): smooth muscle cells, cardiac muscle and pacemaker: inhibitory influence.

Axonen bij somatic nervous system: veel
sterker gemyeliniseerd!

Het parasympatische heeft langere pre-
axonen dan sympathische.

1.3 Autonome controle van de hartslag
Intern ritme wordt geregeld door de SA. Deze
kan ritme creëren zonder invloed van de
hersenen!

SA-node ook beïnvloed door =
® Parasympathic via vagus nerve: hartslag
decrease.
® Sympathetic: hartslag increase.

Electrocardiogram (ECG/EKG) voor registratie van de elektrische activiteit
van het hart uitgevonden door willim Einthoven.

P-wave: bloed komt binnen in atria.
QRS-wave: bloed gaat van atria naar
ventrikels met R-piek: activatie ventrikels.
T-wave: relaxatie.

- Heart rate (HR): beats per minuut (bpm) of het aantal R-pieken in 1 minuut.
- Heart period (HP): interbeat interval IBI in msec of de tijd tussen de R-pieken.

Heart rate variability = hoeveel de HP varieert van beat tot beat.
® Vagale invloeden op de SA-node op ademhalingsritme. Enkel deze laat zulke snelle fluctuaties toe in het hartritme.
® Respiratory sinus arrhytmia (RSA): variations in heart rate at respiratory ritme.
o Inspiratie: minder vagale outflow, hart versnelt.
o Expiratie: meer vagale outflow, hart vertraagt.

® Time domain measures: root Mean Square of Successive Differences (rMSSD)
o Represents short term variation of heart rhythm.
o rMSSD ↑ = ↑ vagal input; rMSSD ↓ = ↓ vagal input.

® Frequency domain measures:
- Ultra low frequency (ULF): < 0.00335 Hz (circadian rhythms, other long term changes in heart rhythm)
- Very low frequency (VLF): 0.00336-0.04 Hz (sympathetic + vagal effects, thermo regulation, vasomotoric, …)
- Low frequency (LF): 0.041-0.15 Hz (tonic sympathetic + vagal effects, blood pressure regulation, …)
- High frequency (HF): 0.151-0.40 Hz (vagal input; but not exclusively: also moderated by respiration)

HF zou goede indicator zijn van vagale input!
