HO-2 - Answers - Broad Form. Named peril form (All perils that form the property
insuring agreement are named.)
HO-3 - Answers - Special Form. Combination of broad perils and open perils
(Structures covered on an open peril basis, contents covered on a broad peril basis). If
the cause of loss is not excluded then coverage would apply. The burden of proof to
prove that the loss is excluded is with the insurance company.
HO-4 - Answers - Contents Broad Form (A.K.A tenants or Renters Form). Designed to
cover the contents of a tenant living in a non-owned structure. Provides the named
broad perils.
HO-5 - Answers - Deluxe Form (Open peril coverage for buildings and contents). The
broadest HO form.
HO-6 - Answers - Condominium Unit-Owners Form. Covers the structural portion as
well as the contents. Covered by the broad perils.
HO-8 - Answers - Modified Coverage Form (Provides the least amount of coverage).
Designed to cover dwellings in which the replacement cost value is much greater than
market value. Coverage for buildings and contents are considered on an actual cash
basis value only.
Coverage A - Answers - Dwelling. Covers: Dwelling described on the declaration page,
structures attached to the dwelling, materials/supplies used for construction. Full
replacement cost as long as the insured carries 80% of the replacement cost value. The
insured selects a limit for coverage A.
Coverage B - Answers - Other structures. A structure adjacent to the dwellings not
physically attached. 10% of coverage A for 1-2 family dwellings. 5% of coverage A for 3-
4 family dwellings.
Coverage C - Answers - Personal Property. Covers property owned or used by the
insured. 50% of coverage A for 1-2 family dwellings. 30% of coverage A for 3 family
dwellings. 25% of coverage A for 4 family dwellings.
Coverage D - Answers - Loss of use. 30% of coverage A on HO-2,3,5. 30% of
coverage C on HO-4. 50% of coverage C on HO-6. 10% of coverage A on HO-8.
