1. Abbreviated Standard Survey: -Complaint Investigations
-In an abbreviated standard survey, the survey team will focus on or limit its
investi- gation to the complaint or a particular area of concern.
-Scope limited to relevant areas of facility operation unless the survey suggests
others
- Concerns that the quality of care may have declined as a result of a change
in ownership, management, or the director of nursing
- An abbreviated standard survey is conducted to investigate complaints or to
determine if certain changes (for example, a change in ownership or
management) have caused a decline in the quality of care provided to nursing
home residents.
2. ABBREVIATED SURVEYS- Timing, scope, duration: The timing, scope,
dura- tion, and conduct of a complaint investigation are at the discretion of the
SA.
An exception is made for complaints that involve allegations of immediate
jeopardy to resident health and safety, and complaints that allege actual harm.
Complaints that involve allegations of immediate jeopardy to resident health and
safety are investigated within two working days of receipt.
Complaints may be received where a resident has experienced (or is alleging)
actual harm that does not rise to the level of immediate jeopardy.
In cases where the SA makes the determination that a higher level of actual harm
is present, the investigation is to be initiated within 10 working days of its receipt.
3. ABBREVIATED SURVEY- When to Investigate a Complaint: In an
abbreviated standard survey, the survey team investigates with the goal of
resolving the com- plaint.
If for example a complaint concerns conditions on a certain day (e.g., weekends)
or during a particular shift (e.g., 11 p.m. - 7 a.m.), then the survey team should
investigate on that day or during that time frame.

- The review of staffing & in-service training
- Surveyors to examine consultant contracts, if appropriate
- The review of policies & procedures related to the noncompliance

6. EXTENDED SURVEY- Part 2: The extended survey should be conducted

Advise the facility's administrator that there will be an extended survey conducted,
and an exit conference will then be held at the completion of the survey.
