Health Perception and Health Management Pattern - focuses on collecting data about a person's
perceived level of health and well-being, as well as their practices for maintaining health. It
includes the evaluation of detrimental habits and identification of actual or potential problems
related to safety and health management. Additionally, it assesses the need for modifications in
the home, the need for continued care at home, and the individual's personal opinion on their
health. Other key aspects include history of allergies, last check-up, history of immunizations,
history of surgeries, and current medications.

Nutrition and Metabolism Pattern - assessment focuses on the pattern of food and fluid
consumption in relation to metabolic needs. It includes evaluating the adequacy of nutrient

host defenses, and gastrointestinal issues. Additionally, it involves assessing the condition of the
skin, scalp, and nails, reviewing the patient's diet menu, considering food restrictions based on
disease or religious beliefs, and understanding their food preferences, including likes and
dislikes.
Roles and Relationship Pattern - assessment focuses on an individual's roles and relationships
with others. It includes evaluating role satisfaction, identifying role strain or dysfunctional
relationships, and understanding the person's role within the family. If the individual is
hospitalized, the assessment also considers who will take over their responsibilities.
