colorectal cancer screening - fecal occult blood test: begin age 50
AND
flexible sigmoidoscopy every 5 years', OR
colonoscopy every 10 years, double contrast barium enema every 5 years, or CT colonography
every 5 years
contact precautions - include enteric precautions - shigella, RSV, cellulitis
wound drainage, fecal incont, or other bodily discharges
PPE: gloves, gown, as needed use of masks and goggles
private room preferred; may cohort
disposal of infectious dressing material into nonporous bag
dedicated equipment for the client to disinfect after used
crutches - approximately 3 fingerwidths between the axilla and top of the crutch
position hands on hand grips with elbows flexed at 30 degrees
crutches: non-weight bearing - 1) being in tripod, maintain weight on unaffected leg
2) advance both crutches AND affected leg
3) move unaffected leg forward past crutches
4) advance both crutches and unaffected leg
5) continue sequence
crutches: weight bearing - Move crutches forward about one step's length
,Move "affected" leg forward; level with the crutch tips
Move the "unaffected" leg forward
Continue sequence making steps of equal length
Decreased breath sounds - Abnormal
Very distant sound
Allow only some of inspiration to be heard
Associated with *obstructive lung diseases*, severe congestion, emphysema or hypoventilation
dental assessments - every 6 months
disease prevention - conduct a risk factor assessment
educate clients to follow standards for recommended screenings
identify lifestyle risk factors requiring modification
encourage client to continue health-promoting behaviors
instruct client about preventative immunizations
airborne precautions - measles, varicella, TB, disseminated herpes zoster
PPE: N95 mask
provide private room with negative airflow
keep door closed
apply particulate mask when pt is leaving the room
, avoid placing clients on isolation precautions - in same room with clients who are
immunocompromised, have open wounds, or have anticipated prolonged lengths of stay
blood cholesterol - begin age 20
minimum every 5 years (if no risk factors)
blood glucose - begin age 45
minimum every three years
blood pressure - begin age 20, minimum q 2 years (annually if higher than 120/80)
BMI - begin age 20
each healthcare visit
bruit heard over the aorta - indicates aneurysm is present
bulb suction drain - drainage around the site is a common occurrence
*color* is most important assessment
c diff - contact for duration of illness
staff and visitors must wear PPE upon entry to room
cane - is used on stronger side to provide support to opposite limb
move cane forward 6 to 10 inches, then move the weaker leg forward
