when speaking. Client states, "I lost my balance and fell." Client has skin tears on left forearm
with bruising. No fracture detected upon x-ray results. Skin dressing on left forearm is dry and
intact. Denies pain at this time.

The client complains of a headache, fatigue, and lack of appetite that started two days ago. The
client awoke today with a rash that looked like blisters all over their face, chest, and back. Client
states, "It is beginning to itch and spread." Lung sounds clear to auscultation. S1, S2, and S3
both heard nothing. In all quadrants, the bowel sounds normal. informed the provider of the
findings. Request a rx for antipyretic.
place in negative-pressure airflow room.
Designate a steth to be used only on the client.
Wear N-95.
One month ago: After participating in a recent natural disaster as a member of the disaster
response team, the client presents to the clinic for evaluation. Medical history significant for type
1 diabetes mellitus. Client did not sustain any physical injuries during the disaster response. "I
still haven't processed everything that happened," the client says. The patient is alert and
oriented, has a normal heart rate and rhythm, and both lungs respond well to auscultation.
Blood glucose obtained. No concerns today. Client to return to clinic for 1 month follow-up visit.
Condition of concentration. Non-fasting casual blood glucose.
