NURS 6521 Case Study - Complex Regional Pain Syndrome
Complex regional pain syndrome (CRPS) is a debilitating disorder that typically affects the limbs after a trauma or nerve injury. As the name of the condition suggests, the physiologic alterations brought on by the triggering lesion are complicated. The CRPS-related pain and disability frequently result in psychological co-morbidities that feed a vicious cycle of agony, loneliness, and depression. We examine current advances in the knowledge of CRPS and the treatment of this illness. It is hoped that additional studies aimed at processes implicated in the pathophysiology of CRPS may result in its prevention (Shim et al., 2019). Case study summary The client is a 43-year-old Caucasian man who presented with the chief complaint of hip pain. According to the client, the pain began seven years ago after a fall at work, and it was later discovered that the cartilage surrounding the right hip joint was 75% torn. He’s had multiple diagnostic scans, including MRIs, CT scans, and x-rays. Although the client is alert and oriented to person, place, and time, denies suicidal thoughts or ideations, and no mental state abnormalities are noted, his physician believes he’s seeking narcotics for a pain that’s “all in his head.” The client reports a euthymic mood for mental status. “Complex Regional Pain Disorder is the official diagnosis (reflex sympathetic dystrophy). Three decision recommendations. Nerve pain is the most challenging pain to diagnose and treat, according to NINDS (2022). Factors that might affect the pharmacokinetic and pharmacodynamic processes will be taken into consideration, but the three treatment recommendations I would start with are: 1. Rehabilitation and physical therapy 2. Spinal cord stimulation 2 3. Medications These decisions are supported by evidence-based literature 1st decision is rehabilitation and physical therapy: These are the most essential CRPS treatments. Maintaining flexibility, strength, and function while keeping the painful limb or body part moving promotes blood flow and reduces circulatory symptoms. Rehabilitation of the injured limb aids in the prevention or reversal of secondary spinal cord and brain alterations caused by disuse and chronic pain. Occupational therapy can assist patients in learning new techniques to become more active and return to employment and daily activities (Price, 2019). 2nd decision is spinal cord stimulator implants. Stimulating electrodes are threaded through a needle into the spine outside the spinal cord. They create tingling sensations in the painful area that help block pain sensations and normalize signaling into the spinal cord and brain. Electrodes can be placed temporarily for a few days to assess if stimulation is likely helpful. Minor surgery is required to implant the stimulator, battery, and electrodes under the skin on the torso. Once implanted, stimulators can be turned on and off and adjusted with an external controller. And 3rd decision is medications. Several medication classes have been reported to be effective for CRPS, mainly when administered early in the disease. However, none have been approved by the FDA to be explicitly marketed for CRPS, and no single drug or combination is guaranteed to be effective in everyone. 1. Gabapentin: Gabapentin (Neurontin®, Pfizer) is an anticonvulsant with analgesic properties in various neuropathic pain syndromes.
