(2020) state that healthcare provider involvement can contribute to safe and efficient care transitions. It is the health care provider's responsibility to establish a care plan that includes patient and family education, effective communication, and transfer of information to other facilities or providers about all appropriate information regarding the patient's health status, considerations, and medications and to facilitate the continuity of care by establishing follow-up appointments (Earl et al., 2020).
