Dit BPV-plan heb ik geschreven voor een ziekenhuisstage in onderwijseenheid 9/10 Samenwerken in Vakmanschap (SIV). Dit was een stage van 20 weken. Hierin staan leerdoelen bij verschillende canmeds rollen (zie inhoudsopgave). Daarnaast staat een plan van aanpak bij elk doel en wat ik nodig heb van mijn stageplek om het doel te behalen. Het plan is goedgekeurd door mijn stagebegeleiders. Uiteindelijk heb ik een 8,5 gehaald voor deze stage met behulp van het werken aan deze leerdoelen.
