Group 1 - two 25 minute contacts per
month w/ care team
Visits of GPS groups
Group 2 - (higher acuity) - six 40 minute
contacts per month w/ care team
Health home manager
Nurse care manager
Staffing of My Health GPS Care coordinator

Licensed clinical pharmacist
