PURPOSE OF MEDICATION
Expected Pharmacological Action Therapeutic Use
Inhibit COX's action Reduce inflammation and fever
COX 1 release prostaglandins, protect gastric mucosa,
enhance platelet aggregation, promote renal function Analgesia for mild to moderate pain
COX 2 release prostaglandins in response to injury which inhibits platelet aggregation
results to inflammation, pain, fever
Complications Medication Administration
Salicylism, Reyes syndrome, GI discomfort (abd pain, Infuse IV ibuprofen for 30+ min,
headache, nausea, dyspepsia), impaired renal function, pt should be hydrated before
increased risk of heart attack and stroke infusion to prevent renal damage
Contraindications/Precautions
Pts hypersensitivity to NSAIDs
Peptic ulcer disease
Bleeding d/o (hemophilia and vitamin K deficiency) Nursing Interventions
Pregnancy category D Observe for GI bleed,
administer proton pump inhibitor
to reduce ulcers, caution w/ pts
w/ heart failure, monitor I&O
BUN and Cr, use smallest dose
Interactions for cardiovascular pts
Use NSAIDs cautious w/ older adults, smoke, H. pylori
infection, hypovolemia, asthma, chronic urticaria, bleeding
d/o, pts taking ACE inhibitors and ARBs
Ibuprofen decreases antiplatelet effects of low dose aspirin
Anticoagulants + alcohol -> increase risk of bleeding
Client Education
Take w/ food or 8 oz water/milk
Tell provider about any OTC
Evaluation of Medication Effectiveness meds, vitamins, or herbals
Reduced inflammation supplements before taking them
Reduced fever Re
Relief mild to moderate pain
Absence of injury
