"I developed a new rash on my arms, chest, and back 3 days ago. It's itchy."



History of Present Illness (HPI):

Onset: Rash developed approximately 3 days ago



Location: Predominantly on the upper arms, chest, and upper back

, Quality: Pruritic (itchy), maculopapular in nature



Duration: Persistent for 3 days, no relief



Associated Symptoms: Some fatigue, but no fever, headache, or muscle aches



Aggravating Factors: Worse with heat or friction from clothing



Relieving Factors: None noted, has tried over-the-counter hydrocortisone cream without significant
benefit



Previous Episodes: Denies any history of prior similar rash or dermatologic issues



Past Medical History (PMH):

General Health: Basically healthy, no chronic illness or major surgery
