Chief Complaint:
“I’m peeing a lot and it hurts.”
History of Present Illness (HPI):
A 16-year-old female presents with a 2-day history of dysuria, increased
urinary frequency, and mild suprapubic discomfort. She denies fever,
chills, flank pain, nausea, vomiting, or vaginal discharge. This is her first
episode of urinary symptoms.
Sexual History:
Sexually active, inconsistent condom use
No contraceptive pills
No history of STIs
Past Medical History:
No chronic illnesses
Immunizations up to date
