Chief Complaint (CC): Smoker with chest tightness
History of Present Illness (HPI):
Steven Van Dyke is a 36-year-old male who presents with
complaints of chest tightness that has been worsening
over the past several weeks. He describes the tightness as
a squeezing sensation in the center of his chest, occurring
mostly during physical exertion and sometimes at rest.
The pain does not radiate but is accompanied by mild
shortness of breath.
He reports a 15-year history of cigarette smoking,
averaging one pack per day. He denies any recent upper
respiratory infections, fever, or cough but admits to
occasional wheezing.
, Past Medical History (PMH):
• Chronic smoker (15 pack-years)
• No known history of hypertension, diabetes, or high
cholesterol
• No prior history of heart disease or lung conditions
Family History (FH):
• Father: Hypertension, died of myocardial infarction
at 58
• Mother: No known cardiovascular disease
• No known genetic lung diseases
Social History (SH):
• Smoker: 1 pack per day for 15 years
• Occasional alcohol use
• No illicit drug use
• Works a desk job with limited physical activity
Review of Systems (ROS):
• Cardiovascular: Chest tightness with exertion, no
palpitations, no syncope
• Respiratory: Mild shortness of breath, occasional
wheezing, no productive cough
• Gastrointestinal: No acid reflux, nausea, or
vomiting
• Neurological: No dizziness, headaches, or numbness
