Root Cause Analysis The general purpose of a Root Cause Analysis (RCA) is to provide information that occurred during an adverse or detrimental event. Ideally the RCA should occur within 45 days of the event. Sifting through the information and the events that occurred from the start to finish of the event help point out the flaws that occurred in order to help prevent an adverse event from happening again. RCA’s look back at all the events that occurred in a retrospective approach. To formulate an RCA all individuals involved in the event should be present. There should be a leader organizing the RCA and each step in the adverse event should start from the beginning of the event and go in chronological order to the end of the event. Each individual involved is expected to be open and honest during this process to not place blame in the situation but to find the root cause of this issue and focus on the system as a whole. The goal is to find the cause of the event, which in most cases in health care is never a single cause. It is almost always a cascade of events that happen, so gathering clear and concise information on the cause helps form a solid RCA to help prevent incidents of such matter from happening in the future.
