For subsequent visits, the record must contain:

2. purpose of the visit and any changes in symptoms
3. an examination of the area involved in the diagnosis
4. an assessment of any change since the last visit; the treatment
or service provided and the patient's response; and any change in
the treatment plan or referrals.




A chiropractor must make sure a patient record is:
1.created timely
2.accurately dated
3. readable by a third party
4.signed or initialed by the chiropractor or the person who
actually performed the treatment or service,
5. contains a key to any abbreviations, including those of
employees (and their professional certification and education, if
any).

a patient (or other person legally authorized to act on the
patient's behalf) must make a request for records in:

information or records the patient wants.


-patient can withdraw the request in writing at any time. The
rule permits you to honor a verbal request if certain conditions
are met. And patients never need a subpoena to request records.




There are also provisions in the rule that allow a chiropractor to
withhold records if the chiropractor believes disclosure could:
harm a patient.
