A1. Initiatives
At Fielder Medical Center, several healthcare quality improvement initiatives are most
effective and efficient in healthcare, such as rescuing patients with severe sepsis and septic
shock, Reducing avoidable readmissions, and central line-associated bloodstream infections
(CLABSI). Since implementing these initiatives at Fielder's Hospital, the quality of care and
treatment patients receive has been significantly improved.
In the past, Sepsis has been proven to be life-threatening and is caused by bacterial infections
that may result from other diseases, such as viruses, parasites, or fungi (CDC News Room,
, 2016.) Sepsis is an inflammatory response caused by chemicals released into the bloodstream to
fight the injection. When sepsis becomes severe enough, this could cause organ failure and a
significant drop in blood pressure. The best way to combat sepsis is through “bundles,” which
help identify and treat anyone infected with sepsis. Since bundles have been used, mortality
related to sepsis and septic shock has been reduced.
The second healthcare quality improvement initiative is reducing avoidable readmissions
through a safe transition from a hospital back home or skilled nursing. These patients typically
have Medicare as insurance and are between 60-100. When a patient has to be readmitted to the
hospital, this causes mental and emotional stress, as well as financial burdens, on a patient
because, with Medicare, they only get two or three months to stay in the hospital for an entire
year. When the patient meets that day, it causes a financial burden on the patient. To prevent
readmissions, family members and patients must be adequately educated, which can be done
through care conferences. During these care conferences, discharge plans and concerns are
addressed, which includes medication education, support the patient and family will have during
