History of Present Illness (HPI)

Chief Complaint:
The patient, a 62-year-old male, presents to the outpatient clinic with increasing complaints of
frequent urination. This symptom has been ongoing for several months and has progressively
worsened. He describes the sensation of needing to urinate "all the time," both during the day
and especially during the night, which has significantly impacted his quality of sleep. He
specifically reports waking up 3-4 times during the night to urinate, a condition known as
nocturia. This is causing him to feel tired and fatigued during the daytime.




History of Present Illness (HPI) Questions and Responses

1. When did you first notice the frequent urination?
"I first noticed it about 3 months ago, but it has been getting worse over the past few
weeks. It’s becoming more disruptive, especially at night."

2. How long has this problem been occurring?
"It’s been happening for a few months now. I’d say around 3 months, but it has been
progressively getting worse recently, especially the nighttime waking."
