Advanced Professional Nurse Advocates for Community vs. Individual Patient

The advanced practice nurse (APN) advocates for at-risk populations in the community

as well as at the individual patient level. Advocating for the patient as well as the community

are equally important, however there are differences. One main difference is the way the APN

can identify the issues affecting the individual patient versus determining the social

determinants of health for an at-risk community (Misto, et al., 2020). An APN advocating for

the individual focuses on personal needs, goals, and preferences of that patient. The goals,

issues, and needs of a patient are addressed by the individual patient and perhaps other team

members (i.e. physicians, social workers, other nurses) directly caring for that patient. The APN

advocating for the community must research and identify the social determinants of health

affecting a community, focusing on the needs and goals of an entire at-risk population. This

includes research from many different sources throughout the community, including collecting

data to determine the needs of the community.

Another difference in the way the APN advocates for individuals in comparison to at-risk

populations is the process to get the suggested changes implemented in the community.

Advocating for the individual patient/client includes establishing the plan of care for that

patient’s individual needs, involving professionals directly involved with that patient’s care. In

comparison, the APN advocating for the at-risk community involve the key stakeholders and

policymakers in that area to change policy. Legislation and other governing entities will likely

have to be involved to accomplish goals to improve the social determinants for the at-risk

community. The APN must determine the key stakeholders and identify policymakers to get
