HEAD AND FACE
1. I will begin with inspec1on of the facial skin and I do not see any discolora1ons or
lesions.
2. I will inspect the pa1ents head, which is a normal size, is symmetric, and in midline
posi1on.
3. Next, I will palpate the lymph nodes of the head and neck:
a. Star1ng with the preauricular lymph nodes in front of the ear
b. The postauricular lymph nodes behind the ear
c. The occipital lymph nodes at the back of the head
d. The tonsillar lymph nodes
e. Coming under the jawline to palpate the submandibular and submental lymph
nodes.
f. Next, I will palpate the anterior cervical lymph nodes and the posterior cervical
lymph nodes.
g. Lastly, I will palpate the supraclavicular lymph nodes right above the clavicles.
h. I do not feel any enlargement.
4. Next, I will test the trigeminal nerve which is cranial nerve #5.
a. First, I will test the motor component by palpa1ng over the masseter muscle as
the pa1ent clenches the jaw.
b. I do not feel any distor1ons and I note great strength.
c. Second, I will test the sensory por1on of the trigeminal nerve by asking the
pa1ent to close their eyes and let me know when and where they feel my touch.
i. Forehead
ii. Right cheek
iii. Left cheek
iv. Chin
v. Nose
5. Next, I will test the facial nerve which is cranial nerve #7 by asking the pa1ent to do
some facial expressions.

e. Pucker lips
f. These expressions are all symmetrical.
EAR
6. I will move on to ear inspec1on, inspec1ng the outer ear for any skin lesions and
symmetry. I do not note any nodes or lesions and the ears are symmetrical.
7. Next, I will inspect the auditory canal and tympanic membrane using the otoscope.
a. I am going to pull your ear up and back, looking into the external auditory canal.
b. There is no cerumen, no swelling or redness, and no drainage.
c. The tympanic membrane is pearly gray and there is no effusion in the middle ear.
d. I will do the same for the other ear.
8. Next, I will palpate the pinnae for any nodules or tenderness. I do not feel any nodules.
Do you feel any tenderness? I will palpate the tragus for any nodules or tenderness. I do
not feel any nodules. Do you feel any tenderness?
9. Next, I will test the acous7c nerve which is cranial nerve #8.
a. I will test hearing by performing the whisper test.
b. Cover one of your ears and I will stand behind, covering my mouth and
whispering three words, please repeat them back to me – 1, 2, 3.
c. Cover your other ear and I will stand behind you, covering my mouth and
whispering another three words, please repeat them back to me – 4, 5, 6.
d. Hearing is intact bilaterally.
