Case Comprehensive Case 26 Year Old Kathleen Parks More Frequent Sereve Headaches 1. History Chief Complaint: The patient is a 26-year-old male with a history of more frequent severe headaches. History of Present Illness: Onset: Ask when the headaches began. Was this increase in frequency recent or related to a sudden onset? Duration: The length of each headache episode, in hours or days Frequency: Frequency of headaches increased from a baseline, such as once a month to twice a week. Location: Unilateral or bilateral? Is pain localized behind the eyes or the forehead or diffuse? Quality of Pain: Ask the patient to describe headache for example, throbbing, stabbing, pressure-like, dull. Severity: Assess severity on a scale of 1-10 pain. Are headaches disabling? Precipitating Factors: What have been the precipitating factors for headaches-stress, dehydration, diet, lack of sleep, menstruation? Alleviating Factors: What helps in the relief of headaches-rest, medications, dark rooms, hydration? Associated Symptoms: Nausea, vomiting, photophobia, phonophobia, or visual aura should be sought. Medication History: What OTC medications, prescription meds, and supplements is the patient currently taking? Keep in mind medication overuse headache, especially with frequent use of NSAIDs, triptans, or opioids. Impact on Life: How are these headaches influencing daily
