What are some examples of therapeutic communication the nurse should use when communicating with this client? -Correct Answer-Relate therapeutically to the client who is unable or unwilling to communicate. Make time to be with the client, even if he does not speak. Make observations rather than asking direct questions, which can cause anxiety in the client. For example, the nurse might say, "I noticed that you attended the unit group meeting today," rather than asking, "Did you enjoy the group meeting?" Give directions in simple, concrete sentences because a client who has depression can have difficulty focusing on and comprehending long sentences. Untreated depression increases the risk of suicide in the older adult client. Other risk factors for the older adult client include loss of employment and finances, feelings of isolation, powerlessness, prior attempts at suicide (older adult clients are more likely to succeed), change in functional ability, alcohol or other substance dependency, and loss of loved ones.(Mental Health RM Chp 30) What are the phases of the cycle of violence that can occur between domestic partners and what may take place during each of these phases? -Correct Answer-Tension-building phase - The abuser has minor episodes of anger and can be verbally abusive and responsible for some minor physical violence. The victim is tense during this stage and tends to accept the blame for what is happening. Acute battering phase - The tension becomes too much to bear and serious abuse takes place. The victim can try to cover up the injury or can get help. Honeymoon phase - The situation is defused for awhile after the violent episode. The abuser becomes loving, promises to change, and is sorry for the behavior. The victim wants to believe this and hopes for a change. Eventually, the cycle begins again. Periods of escalation and de-escalation usually continue, with shorter and shorter periods of time between the two. Emotions for the abuser and victim, such as fear or anger, increase in intensity. Repeated episodes of violence lead to feelings of powerlessness.(Mental health RM chapter 32) A nurse suspects a client may be experiencing acute mania.
